Low Vision Aids

[:en]Where good vision cannot be achieved with spectacles or contact lenses, patients can be assessed in our low vision clinic.
During a low vision assessment, one of our optometrists will perform an examination to identify any refractive errors (problems with focusing) and demonstrate low vision devices such as specialist spectacles, magnifiers or telescopes that could help you see better.
Electronic magnifiers are also available for demonstration, and the optometrist will also talk to you about optimal lighting and other methods to make the most of your remaining vision.[:ar]عندما لا تتحسن قدرة الإبصار بواسطة النظارات أو العدسات اللاصقة، فإن بالإمكان تقييم حالة المرضى في عيادة علاج الرؤية المتدنية.

Paediatric Cataract

[:en]Our paediatric team can manage complex congenital cataracts. The management of congenital cataract is very different to the treatment of a routine age-related cataract. In adults, surgery may be delayed for years without affecting the visual outcome. In infants, if the cataract is not removed during the first year of life, the vision will never be fully regained after surgery. We are able to perform lensectomy, intraocular lens implants on children above the age of one year, secondary intra ocular lens implants and surgery for dislocated lenses.[:ar]يتولى طاقمنا الطبي المتخصص بطب عيون الأطفال علاج حالات إعتام عدسة العين الخلقية المعقدة. وتختلف طريقة التعامل مع إعتام عدسة العين الخلقي إلى حد كبير عن علاج الحالات المعتادة المرتبطة بتقدم السن. فالجراحة لدى الكبار يمكن تأجيلها لسنوات دون أن تتأثر النتيجة من حيث قدرة الإبصار. أما لدى الأطفال وحديثي الولادة فإنه لا يمكن استعادة البصر بشكل كامل بعد الجراحة في حال لم يتم التخلص من الإعتام خلال السنة الأولى من عمر الطفل. ويمكن لفريقنا الطبي إجراء عمليات استئصال العدسة وزراعة العدسات الصناعية للأطفال فوق عمر السنة، بالإضافة إلى زراعة العدسات الثانوية وجراحة حالات خلع العدسة.[:]

Anti-angiogenic (anti-VEGF) drugs

[:en]This treatment involves having an injection into your eye to treat certain retinal conditions that cause abnormal blood vessels to grow and leak under the retina. Patients with these conditions can lose central vision when abnormal blood vessels bleed under the retina at the back of the eye. A series of injections of anti-VEGF medicines are given into the back of your eye to stop these blood vessels growing and help control the leaking blood. This treatment is highly effective in preserving central vision in many people.
Common conditions in which it may occur are Wet age-related macular degeneration, Myopic choroidal neovascularization, Diabetic macular oedema, Retinal vein occlusion and any other retinal conditions that causes fluid to leak under the retina.[:ar]يتضمن هذا العلاج حقن العينين لعلاج حالات معينة في الشبكية قد تتسبب بتشكل أوعية دموية جديدة تنمو وتنزف تحت الشبكية. وقد تؤدي هذه المشكلة إلى فقدان الرؤية المركزية لدى بعض المرضى وذلك عندما تنزف الأوعية الدموية الجديدة تحت الشبكية في مؤخرة العين. وفي هذه الحالة يتم إعطاء المريض مجموعة من حقن الأدوية المضادة لتشكل الأوعية الدموية الجديدة في مؤخرة العين للمساعدة بالسيطرة على النزيف. هذا العلاج فعال جداً في الحفاظ على الرؤية المركزية لدى العديد من المرضى.
ومن الحالات الشائعة التي قد يحدث فيها تشكل لأوعية دموية جديدة مرض الضمور البقعي الرطب المرتبط بتقدم السن، واتساع الأوعية الدموية المشيمية في حالات قصر النظر، والوذمة البقعية الناجمة عن مرض السكري، وانسداد الوريد الشبكي، وغيرها من مشاكل الشبكية التي تتسبب بتسرب السوائل تحت الشبكية.[:]

Dacrocystorhinostomy

[:en]This is an operation to form a new tear drain between your eye and nose when there has been a blockage. If the normal drainage passage gets blocked or narrow, you might get a watery eye or repeated eye infections.
During DCR, your tear drainage passages are opened so that the tears can drain into the nose. There are two approaches to DCR surgery: external DCR and endoscopic , endonasal DCR, often referred to as EndoDCR. They are similar operations with similar success rates.
External DCR
The operation takes place through a 10- 15mm incision in the side of your nose, where a pair of glasses would rest. This heals up very quickly and is rarely visible when healing is complete in most people. As part of the procedure, small polythene rods (often referred to as tubes, although they do not assist with drainage) are positioned internally to ensure that the newly made passage remains open during the healing phase. These rods are removed after four to six weeks.
Endoscopic DCR
Endoscopic DCR is a minimally invasive procedure to unblock tear ducts. The operation is very similar to external DCR, except there is no cut through the skin and no scar afterwards. Access is through your nose, using a small thin camera (endoscope). As with all DCR surgery, small rods will be placed internally, positioned in the newly created passage and these will be removed after four to six weeks.[:ar]هذه العملية تهدف إلى تشكيل مجرى دمع جديد بين العين والأنف عند حصول انسداد في مجرى الدمع الأصلي. ففي حال انسداد أو تضيق مجرى الدمع الطبيعي فإن المريض قد يعاني من عيون مدمعة أو من تكرار التهاب العين.
ويتم خلال عملية توسيع مجرى الدمع (DCR) فتح مسار مجرى الدمع بحيث يتم تحويل الدموع عبر الأنف. ويتم إجراء هذه العملية بواحدة من طريقتين: عملية توسيع مجرى الدمع الخارجية، وتوسيع مجرى الدمع بالمنظار عبر الأنف وتسمى عادة EndoDCR. وتتشابه هاتان العمليتان كما تتمتعان بمعدلات نجاح متشابهة.
عملية توسيع مجرى الدمع الخارجية
يتم إجراء العملية عبر شق جراحي بطول 10-15 مم على جانب الأنف، في مكان وضع النظارات. يشفى هذا الجرح بسرعة كبيرة ولا يكاد بظهر له أثر بعد التئامه لدى العديد من المرضى. وخلال هذه الجراحة توضع قضبان صغيرة من البوليثين (ويطلق عليها اسم أنابيب إلا أنها لا تلعب دوراً في عملية تصريف الدمع) لضمان بقاء المجرى الجديد مفتوحاً خلال مرحلة الشفاء. تتم إزالة هذه القضبان بعد فترة 4-6 أسابيع.
توسيع مجرى الدمع بالمنظار
توسيع مجرى الدمع بالمنظار هو إجراء يتطلب حداً أدنى من التدخل الجراحي ويهدف إلى فتح قنوات الدمع المسدودة. تشبه هذه العملية إلى حد كبير عملية توسيع مجرى الدمع خارجياً، إلا أنها لا تتطلب إحداث شق جراحي ولا تترك ندبة على الجلد. يتم الوصول إلى القنوات عبر الأنف باستخدام كاميرا صغيرة (هي المنظار). وكما هو الحال في كافة جراحات توسيع مجرى الدمع يتم وضع القضبان الصغيرة في الممرات التي تم تشكيلها خلال العملية، وتتم إزالتها بعد 4-6 أسابيع.

Cataract

[:en]Having a cataract is a bit like having a dirty windscreen on a car. It can make the view cloudy or foggy or sometimes blurry. It can also cause glare from bright lights, sunshine, and on-coming headlights while driving. When these problems make your normal daily activities difficult, it is time consider the cataract operation. Modern small incision cataract surgery is highly successful in some patients. Cataracts do not permanently impair your vision and you will not go blind from a cataract. An operation will restore your sight. Most forms of cataract develop in adult life, and usually occur as we get older.
In the majority of cases we can remove the need for distance glasses and you may only need a pair of reading glasses. We also use multi intraocular lens implants they can give vision at both distance and near. Whilst trifocal intraocular lenses give vision at near, intermediate and distance. Although these may not be suitable for everyone, our doctors will assess you for the most suitable lens and multifocal IOLS to achieve spectacle independence.
[:ar]الإصابة بإعتام عدسة العين شبيه بزجاج سيارة أمامي غير نظيف. فهي تجعل الرؤية مشوشة أو ضبابية. كما يمكن لإعتام عدسة العين أن يسبب وهجاً شديداً نتيجة التعرض للأضواء الساطعة وضوء الشمس وأضواء السيارات في الاتجاه المقابل أثناء القيادة. عندما تتسبب هذه المشاكل في جعل الأنشطة اليومية العادية أمراً صعباً، فإن على المريض أن يدرس خيار إجراء عملية لعلاج إعتام عدسة العين. إن الجراحة الحديثة لعلاج إعتام عدسة العين عبر شق جراحي صغير تحقق نجاحاً كبيراً لدى بعض المرضى. لا يتسبب إعتام عدسة العين في إعاقة البصر بشكل دائم ولن يصاب المريض بالعمى من إصابته بهذه الحالة. وتساعد العملية الجراحية على استعادة المريض لقدرة الإبصار. يذكر أن معظم حالات إعتام عدسة العين تصيب الأشخاص البالغين، وعادة ما تحدث عند التقدم في السن.
وفي معظم الحالات يمكن أن يتخلص المريض من نظارات الرؤية عن بعد بحيث يحتاج فقط إلى نظارات القراءة. كما أننا نستخدم عدداً من عدسات العين الصناعية التي يمكن عند زراعتها من تحسين الرؤية القريبة والبعيدة. ومن جهة أخرى فإن عدسات العين ثلاثية البؤرة تحسن قدرة الإبصار القريبة والمتوسطة والبعيدة. وعلى الرغم من أن هذه العدسات قد لا تناسب الجميع إلا أن أطباءنا سيقومون بتقييم حالتك لتحديد العدسة الأنسب.[:]

Our 200 Years Heritage

At the beginning of the 19th century, ophthalmology was an unknown science. That all changed in the early 1800s as many soldiers returned from the Napoleonic wars suffering with trachoma. With the epidemic as an impetus, and coupled with the encouragement of the influential London surgeon Astley Cooper, John Cunningham Saunders founded the first hospital in the world devoted to the treatment of eye disease in October 1804.
The number of patients seeking treatment steadily increased, forcing a move to a larger site in 1822 and finally to Moorfields present main site on City Road, central London, in 1899. At this time, the first specialist departments were set up (x ray and ultra violet treatment rooms) and the hospital was still operating as a charity where each patient received an admission card that read: “This letter is granted to the applicant in being poor. Its acceptance therefore by anyone not really poor constitutes an abuse of charity.”
In 1935, after a public appeal for £120,000, the George V extension was opened. This provided a new outpatient department, a new ward and an orthoptic department. During the Second World War, the hospital opened its doors to general surgical cases and most of the ophthalmic patients were evacuated out of London. Moorfields received a direct hit from a “doodlebug” in 1944 and suffered serious damage. This was so extensive that the hospital was nearly pulled down and rebuilt on a green field location. However, the site was rebuilt and in 1946 the City Road Hospital amalgamated with the Royal Westminster Ophthalmic Hospital, the Central Eye Hospital and took on the clinical facilities of the medical school for the University of London.
With the formation of the NHS in 1948, Moorfields lost its status as a voluntary hospital and in 1956 was officially named as “Moorfields Eye Hospital”. In 1950, the Sixteenth International Congress of Ophthalmology was held at Moorfields and for the first time ever TV cameras were installed in the theatres to demonstrate surgical techniques. Moorfields was redeveloped in the late 1980s allowing for the expansion into more specialist areas. The Trust responsible for Moorfields London now runs outreach community eye clinics at nine other sites where it provides a wide range of ophthalmic services.
Throughout its history Moorfields has been at the forefront of pioneering eye care and it remains one of the largest centres for ophthalmic treatment, research and training in the world. More patients in the UK go to Moorfields than to any other eye hospital or clinic because of its world-famous reputation. This is based on the expertise of its clinical staff and the cutting-edge research taking place. In fact, over half the ophthalmologists practising in the UK, and many more overseas, have received specialist training at Moorfields.
